I/We assume all risks of injury from any activity connected with Camp 43. We do not find Camp 43/Landowner liable.
I/We the parent/guardian of the above named child, give approval for any film videotape and photographs of him/her to be used for promotion or information purposes of Camp 43. I/We give permission for him/her to participate in Camp 43.
